12th Grade Immunization Requirement for School Attendance

A school document detailing immunization requirements for seniors is presented against a plain white background.

The Ohio Department of Health (per Ohio revised code 3313.67) REQUIRES ALL STUDENTS ENTERING THE 12th GRADE to have:

A second dose of the Meningococcal vaccine (Serogroup A,C, W, and Y) (If your student received their first dose on or after their 16th birthday, a second dose is not required).

The record should be received no later than the 14th day of school:

September 17, 2026

As with other school immunization requirements,

Students without proof of required immunizations after this date are subject to exclusion from school

This INCLUDES seniors that attend MCCTC or YSU campuses Please contact your child’s physician or the Mahoning County District Board of Health to make an appointment at (330)270-2855 ext. 125. Immunizations are also provided at many local pharmacies. If you have an appointment scheduled for your immunization, notify me of this date & your student will be considered in-progress.

Please forward the record to the school office when your child receives his/her immunization and it will be placed in their health file. It may also be attached to Final Forms, emailed to the nurse hlefoer@polandschools.org, or faxed directly to the Poland Schools confidential clinic line at (330)757-2095.

If you have any questions, please contact Holly Lefoer, School Nurse, at the above email or (330) 757-7018, prompt #6.

**Please note: There is also a vaccine for Meningococcal Serogroup B which does not satisfy or replace the state vaccine requirement. This is a different and optional vaccine**
