7th Grade Immunization Requirements for School Attendance

Dear Parents of Incoming 7th Grade Students, 

The State of Ohio 

REQUIRES ALL STUDENTS ENTERING THE 7th GRADE to have received the following immunizations: 

1) Tdap booster (Tetanus, Diphtheria, Pertussis) 

2) Meningococcal vaccine (Serogroup A, C, W, and Y) 

If this has not already been completed, we are encouraging all upcoming 7th grade students to have an appointment scheduled with their Healthcare Provider prior to the first day of the 2024-2025 school year. 

As with other school immunization requirements, students without proof of required vaccines are subject to exclusion from school if verification is not received by the 14th day of school: September 19, 2025. 

In addition to your child’s Healthcare Provider, The Mahoning County District Board of Health offers vaccination administration. To make an appointment, call (330)270-2855 ext. 125. They are also available at many local pharmacies. 

Please forward the record to the school office when your child receives his/her vaccination and it will be placed in their health file. It can also be 

faxed to the clinic at (330)757-2095.
