Thanks to Dr. Black, Kirstie Bournias, & Mercy Health, physicals will be provided free of charge to all Poland athletes! Most schools do not offer this free of charge, so please show your appreciation to these individuals for their generosity!
In order to take advantage of the free physical, you MUST have the printed out physical WITH your parent signature on all relevant lines. If you do not have this sheet with you, or it is not signed, you will not be able to get your physical!
(In FinalForms, look for "OHSAA PPE Physical Form". It will say "Click to Print".)
PLEASE COMPLETE ALL FINALFORMS BEFORE PRINTING!
UPDATE: Physicals will be held in the Teachers' Lounge. Please enter through the field house lobby and go through the cafeteria to the Teachers' Lounge.
Please arrive for your physical 5 minutes prior to the start of your scheduled time block.Please sign up with your child's name and child's Poland email address. These will be used to send out the baseline impact concussion test.
IF YOU NEED A CONCUSSION TEST you will get an email from Athometest@impacttest.com. There will be a link to click on for directions. Please watch the video then click on the link to take the test. The test will take about 30-40 minutes to complete and must be completed on a computer or chrome book. The test is not compatible with cell phones. The concussion test will NOT take place on the day of physicals.
Parents do not need to accompany you or stay with you during this physical. You should allow approximately 30 minutes for the physical. If you have any questions, please call or text Kirstie Bournias at (412) 862 - 7875, or email her at KBournias@polandschools.org.
