Free & Reduced Price Meals Application

The National School Lunch and School Breakfast Program ensures all students have access to meals during school, specifically students who cannot pay the full meal price. Federal Income Eligibility guidelines determine students’ eligibility for the program. The official policy and guidelines have been published below.

If you are interested in applying for the program, please complete the application form in PaySchools Central. Once you create/login to your account, you will see a link to the "Free/Reduced Meal Application".

The application form is also available below. Completed forms can be returned to your child's school office or sent to Tammi Pearch, Food Services, 3030 Dobbins Road, Poland, OH, 44514.

An optional application for Ohio’s Healthy Start & Healthy Families program is included in the application. Families with children eligible for school meals may be eligible for low-cost health care coverage through Medicaid and/or Ohio’s Healthy Start & Healthy Families programs. These programs include coverage for doctor visits, immunizations, physicals, prescriptions, dental, vision, mental health, substance abuse and more. Information can also be found on the web at https://medicaid.ohio.gov/wps/portal/gov/medicaid/
